Medications


  • Per NYS law, students needing to take either prescription or over-the-counter medication during the school day require the following:

    • A written order from your physician. This must be a FULL order including correct person, med, dose, route, time, and reason.
    • Permission to administer medication, signed by the parent or legal guardian [PARENT/GUARDIAN SIGNATURE NEEDED TO ACTIVATE THE DOCTOR'S SCHOOL-MED ORDER].
    • Medication must be in the original container and given to the nurse by an adult.

    • A Care-Plan from your physician is needed for ANY Chronic Medical Condition [i.e. seizures, diabetes, chronic asthma, etc] for EACH/EVERY school year, as well as any required medication order.

    **All medication must be stored in the Health Office. Epi-pens and Albuterol inhalers are the only medications that MAY be self-carried and administered by the student, if indicated in the doctor's order. The ability to self-carry and self-administer needs to be indicated in the providers' order every school year.

    Students are responsible to come at the appropriate time for medications. The nurse cannot interrupt classes to call students for their medication.

    **PLEASE NOTE:  ALL SCHOOL-MED DOCTOR ORDERS [INCLUDING SELF-CARRY INHALER AND EPI PEN] ARE TO BE RENEWED EACH AND EVERY SCHOOL YEAR. 


     

    Health care providers may write their own order, or use the form provided below. Parents must also sign permission to give the medication. You may sign either the doctor's order, the form below, or a separate sheet of paper.
