• Welcome to Gates Chili High School Health Office

    Health Office Summer Hours:

The health office is staffed limited hours over the summer, which may result in delayed response times. At this time, visitors are restricted to campus buildings and we are NOT open to the public or "walk in" visits. Please call or email with any questions.

Please contact christine_gear@gateschili.org, lauren_goggin@gateschili.org, or call (585) 247-2420 with any questions. Physicals, immunization records, medication orders, etc may be emailed or faxed to (585) 340-5545 over the summer.

Work permits are being issued via an alternative virtual process. Please see the  "Covid-19 Work Permit Alternative Process" link. You may also click here: Work Permits 2020

!!ATTENTION!!: All 12th grade cohort students MUST receive AND provide a record of their Meningitis vaccination, received after their 16th birthday. Failure to do so will result in exclusion from school, per NYS Education law. This includes exclusion from both in-person AND distance learning.

Please contact your child's physician to receive this vaccination and obtain a copy of their record prior to the 1st day of school. Documentation can be faxed to 340-5545 or emailed to the above email addresses. It is parent/guardian responsibility to ensure their child's immunizations stay compliant with NYS law.

Stay safe & healthy and practice social distancing!
