Physical Exams

As of July 1, 2018 New York State requires a physical examination of each child in Grades PK or K, 1, 3, 5, 7, 9 and 11 and all students new to the school district. Students who have an IEP are also required to have a current physical on file with the school before their annual CSE meeting.
These examinations should be done by their primary care physician, since they are more familiar with the health needs of the child and can provide required vaccinations. Due to the ongoing restraints of Covid-19, school physicals performed by the district medical director will only be available on a very limited basis.
These physical examinations must be completed by a NYS licensed physician, physician's assistant, or nurse practitioner on the approved NYSED Student Health Examination Form.
